IncluzIT HOME hosts a Canadian Network of Occupational Therapists who conduct Home Safety Assessments to address safety risk factors and accessibility barriers.  This early intervention can include equipment recommendations, home modifications solutions and community support connection. It is a human-to-human service offered across Canada to enable Actionable Aging™, while preventing falls, hospitalizations and premature admissions to long term care facilities.

IncluzIT HOME is an evidenced-based tool designed by an Occupational Therapist with input from older adults, care partners, service providers and our research partners across Canada.

Below are some of the findings from the study we conducted in 2025 with our research partners at Sheridan College, Centre for Elder Research on the IncluzIT HOME platform priorities and interests, engaging older adults and their care partners across Canada.
